Wegmans is a great choice in the Northeast, and Whole Foods is good as well. Safeway is a pretty solid choice too, believe it or not.
I'd avoid buying fish from places that have ignored sustainable seafood issues despite repeated entreaty from many different groups. Costco is one such place. So is SUPERVALU, the chain that owns Albertson's, Jewel-Osco, and many other banners. Some regional chains are in bad shape too, like Meijer, H.E.B., and Winn-Dixie. Definitely stay away from Albertson's and Costco, they're way behind.
Beyond this, I highly recommend checking out Greenpeace's Carting Away the Oceans report at http://www.greenpeace.org/usa/campaigns/oceans/seafood to find out the full list of supermarkets in your area and how they stack up.
If you're in the San Francisco Bay Area, Andronico's is one of the best places around to buy seafood. The use a program calle FishWise that puts the sustainability rankings (according to the Monterey Bay Aquarium) of each seafood product right there on the wetcase sign -- red, yellow, or green. Excellent transparency, really empowering to the consumer. I like this. Bi-Rite and Natural Grocery do this too.
